This report must include the follow/rig information: <br /> <br />(1) name and address nfthe injured or decea.~ed person(s); (2) name and address of <br />Contzactor's subcontractor, i£any; (3) name and address of Contractor's liabili~' insurance <br />carrier; and (4) a detailed description of accident and whether an}, of ACMC's equipment, <br />tolls, mater/als, or staffwere involved. <br /> <br />I4.
