An ARU representative will provide transportation to Axis Community Health, <br />Pleasanton clinic site, located at 5925 W. Las Positas Blvd. <br />Axis Community Health: Axis will provide the medical physician team, necessary medical supplies, and <br />expertise required to provide medical care. Axis will provide care coordination services such as <br />enrollment into Medi-Cal, Cal -Fresh, and service linkage for community supports, as needed. <br />
