DocuSign Envelope ID: D7EF525D-DBF6-4AO6-BOA8-1570CBCFE8C4 Exhibit A Cost and Term of Service: Starting May 11, 2024, Axis Community Health will provide an allocated physician team, along with support services at a cost of $350 per week. For the duration of the First Amendment, May 11, 2024 to November 9, 2024 (26 weeks), cost will not exceed $10,000. Responsibilities: The City of Pleasanton: Alternate Response Unit (ARU) will provide support in coordinating visits with Axis Community Health. An ARU representative will provide transportation to Axis Community Health, Pleasanton clinic site, located at 5925 W. Las Positas Blvd. Clients will also be able to access care as a walk-in as long as they are referred by the ARU team/representative. Axis Community Health: Axis will provide the medical physician team, necessary medical supplies, and expertise required to provide medical care. Axis will provide care coordination services such as enrollment into Medi-Cal, Cal -Fresh, and service linkage for community supports, as needed.
