EXHIBIT A FY 2025/26 – RATE SHEET <br /> “UTILITIES – WATER, SEWER, & STORM MAINTENANCE <br />& EMERGENCY REPAIRS <br />“UNDERGROUND INFRASTRUCTURE REPAIRS” <br />SCOPE OF SERVICES <br />The city is requesting an experienced licensed contractor to perform routine and emergency <br />services. This work would be performed at any of the City’s water, sewer or stormwater facilities <br />located throughout the city: <br />1. Contractor will repair and/or replace pipes and or structures “as required” on a scheduled <br />and 24-hour emergency basis including overtime if required according to prescribed <br />specifications and City of Pleasanton Standard Details. <br />2. Contractor shall diligently perform the services and furnish the personnel, materials, <br />equipment, vehicles, safety devices, traffic control equipment and plans for pipes and <br />structure repairs and replacement work. <br />3. Contractor, after being advised by the Utilities Division on a work area, must notify <br />residents or businesses in writing a notice to perform services for the city a minimum of <br />48 hours prior to work being started. Contractor must also provide a copy of the notice to <br />the Utilities Division. <br />4. In emergency situations, the contractor must verbally notify residents and businesses prior <br />to interruption of service. <br />5. All necessary asphalt or concrete cutting or sawing removal and replacement per City <br />Specifications. <br />6. Submit to City of Pleasanton Utilities Division as-built drawing for each new project <br />including address and location. <br />7. All OSHA Safety requirements must be adhered to by the Contractor while on-site. <br />RATE SHEET <br />The cost quoted below shall include all taxes and all other charges, including travel expenses, <br />and is the cost the City will pay for the term of any contract that is a result of this bid. <br /> Bidder hereby certifies to City that all representations, certifications, and statements made by <br />Bidder, as set forth in this Bid Form and attachments are true and correct and are made under <br />penalty of perjury pursuant to the laws of California. <br /> <br />Contractor's Labor rates (Hourly) <br />Position Description Regular Time Overtime Emergency Callout <br />Foreperson $ $ $ <br />Operator $ $ $ <br />Page 139 of 264
