Page 7
EXHIBIT A

CITY OF PLEASANTON
HOUSING & HUMAN SERVICES GRANT (HHSG) PROGRAM
BETWEEN

CITY OF PLEASANTON
AND
Hively

JULY 1, 2025 THROUGH JUNE 30, 2026

Project No. 2026098
Hive on Wheels – Mobile Family Resource Center

The project to be undertaken by Hively shall involve:

Hively’s Family Resource Centers are vital one-stop shop community resource and outreach hubs where parents
access not only children’s essentials, but critical family strengthening resource navigation services. Since our first
center opened in Dublin in 2022, we’ve connected with thousands of families seeking services and support and we
continue to strengthen many of those families for the long term through Hively program enrollments. Building on the
success of our established, but fixed-in-place, Hively Family Resource Centers, The Hive on Wheels will expand
our service delivery capabilities, literally putting our family resource centers on wheels, in order to reach into the
area of Tri-Valley communities where families live, work, worship and play.

Stocked with diapers, wipes, clothing, books and children’s essentials and staffed with community resource staff
and family support specialists, the van will station itself at 4 events in Pleasanton over the span of the year (i.e.
community events, churches, recreational areas and other gathering spots).

The project shall conform substantially to the scope of work described above. [However, the targets and service
goals noted in the application may be adjusted proportionately to reflect a reduction in funding between the amount
requested and the amount approved.] Any request to modify the scope of the project shall be submitted to the City
of Pleasanton’s HHSG administrator in the form of a letter. Requests shall be processed as follows:

a) If the proposed change is in substantial conformance with the original project scope as determined by the
City’s HHSG program administrator, the change may be approved by City staff.
b) If the proposed change is in partial conformance with the original project scope as determined by the City’s
HHSG program administrator, the change will be submitted for review and approval by the City’s Human
Services Commission and/or Housing Commission.
c) If the proposed change is not in conformance with the original project scope as determined by the City’s
HHSG program administrator, the change will be submitted for review and approval by the City’s Human
Services Commission and/or Housing Commission and then submitted for approval by HUD in the form of a
change to the Annual Action Plan.
Docusign Envelope ID: 292EDF05-B46C-495A-8549-7F685AE389EEDocusign Envelope ID: 148EF2C1-52E6-4436-AB76-C8A576F0DC5D
