Page 7
EXHIBIT A
CITY OF PLEASANTON
HOUSING & HUMAN SERVICES GRANT (HHSG) PROGRAM
BETWEEN

CITY OF PLEASANTON
AND
Axis Community Health

JULY 1, 2025 THROUGH JUNE 30, 2026

Project No. 2026086
Supporting Axis’ New Livermore Clinic

The project to be undertaken by Axis Community Health shall involve:

Axis Community Health is utilizing $10,000 in grant funding to support the final build-out and operational readiness
of its new clinic at 1686 Second Street in Livermore. This clinic expansion is designed to relieve capacity at existing
Pleasanton facilities, improve regional healthcare access, and serve an estimated 2,500 new patients annually—
primarily from low- and moderate-income populations. The primary objectives of this grant-funded initiative are to
expand regional access to primary and behavioral health care, alleviate capacity constraints at Axis' Pleasanton-
based clinics, and improve health outcomes for underserved Pleasanton residents and the greater Tri-Valley area.

The project shall conform substantially to the scope of work described above. [However, the targets and service
goals noted in the application may be adjusted proportionately to reflect a reduction in funding between the amount
requested and the amount approved.] Any request to modify the scope of the project shall be submitted to the City
of Pleasanton’s HHSG administrator in the form of a letter. Requests shall be processed as follows:

a) If the proposed change is in substantial conformance with the original project scope as determined by the
City’s HHSG program administrator, the change may be approved by City staff.
b) If the proposed change is in partial conformance with the original project scope as determined by the City’s
HHSG program administrator, the change will be submitted for review and approval by the City’s Human
Services Commission and/or Housing Commission.
c) If the proposed change is not in conformance with the original project scope as determined by the City’s
HHSG program administrator, the change will be submitted for review and approval by the City’s Human
Services Commission and/or Housing Commission and then submitted for approval by HUD in the form of a
change to the Annual Action Plan.
Docusign Envelope ID: 1FF54D60-4FAC-4FF9-AE01-6E4CA6FCC839
