Page 7 <br />EXHIBIT A <br /> <br />CITY OF PLEASANTON <br />HOUSING & HUMAN SERVICES GRANT (HHSG) PROGRAM <br />BETWEEN <br /> <br />CITY OF PLEASANTON <br />AND <br />Centro Legal de la Raza <br /> <br />JULY 1, 2025 THROUGH JUNE 30, 2026 <br /> <br />Project No. 2026088 <br />Tri-Valley Fair and Secure Housing Project <br /> <br />The project to be undertaken by Centro Legal de la Raza shall involve: <br /> <br />Our primary goal and objective is to enhance the quality of life for low-income tenants of Pleasanton via housing- <br />related legal services to prevent homelessness and to promote self -sufficiency, contributing to housing stabilization. <br /> <br />To assist with those outcomes, we have set the following performance goals: <br /> <br />Performance Goal 1: 25 Tenants receive legal consultation for housing-related problems that include offering <br />resources and information about tenants’ legal options and rights. <br /> <br />Performance Goal 2: 5 Tenants receive legal representation in housing-related matters. <br /> <br />The project shall conform substantially to the scope of work described above. [However, the targets and service goals <br />noted in the application may be adjusted proportionately to reflect a reduction in funding between the amount <br />requested and the amount approved.] Any request to modify the scope of the project shall be submitted to the City of <br />Pleasanton’s HHSG administrator in the form of a letter. Requests shall be processed as follows: <br /> <br />a) If the proposed change is in substantial conformance with the original project scope as determined by the <br />City’s HHSG program administrator, the change may be approved by City staff. <br />b) If the proposed change is in partial conformance with the original project scope as determined by the City’s <br />HHSG program administrator, the change will be submitted for review and approval by the City’s Human <br />Services Commission and/or Housing Commission. <br />c) If the proposed change is not in conformance with the original project scope as determined by the City’s <br />HHSG program administrator, the change will be submitted for review and approval by the City’s Human <br />Services Commission and/or Housing Commission and then submitted for approval by HUD in the form of a <br />change to the Annual Action Plan. <br />Docusign Envelope ID: 0DD2F98E-54E2-4657-A4D6-38465F59CDBADocusign Envelope ID: 032F6C03-F742-4427-A580-7AFDE1434CE7
