OPTIONAL Community Development Block Grant Certification <br />Submit the following certification only when one or more of the activities in the action plan are designed <br />to meet other community development needs having particular urgency as specified in 24 CFR <br />570.208(c): <br />The grantee hereby certifies that the Annual Plan includes one or more specifically identified CDBG- <br />assisted activities which are designed to meet other community development needs having particular <br />urgency because existing conditions pose a serious and immediate threat to the health or welfare of the <br />community and other financial resources are not available to meet such needs. <br />Signature of Authorized Official Date <br />Title <br />Page 253 of 908
