PLEASE READ IT CAREFULLY. <br />ADDITIONAL INSURED -LANDLORDS, OWNERS, OR PROPERTY MANAGERS <br />WITH DESIGNATED PREMISES AND PRIMARY AND NON-CONTRIBUTORY <br />WHEN REQUIRED BY CONTRACT <br />This endorsement modifies insurance provided under the following: <br />SPECIFIED MEDICAL PROFESSIONS GENERAL LIABILITY INSURANCE COVERAGE PART -CLAIMS-MADE <br />COVERAGE <br />SPECIFIED MEDICAL PROFESSIONS GENERAL LIABILITY INSURANCE COVERAGE PART -OCCURRENCE <br />COVERAGE <br />SCHEDULE <br />Designated Premises: on file with the Company <br />In consideration of the premium paid, it is hereby understood and agreed that the policy is amended as follows: <br />A.Solely with respect to the Specified Products, Goods or Operations of the Named Insured shown in Item 4. of the <br />Declarations, Section The Insured is amended to include as an additional insured any landlord, owner, or property <br />manager of the Designated Premises shown in the Schedule of this endorsement to whom the Named Insured is <br />required by lease, or written contract or agreement, to provide such additional insured coverage. Such landlord, <br />owner, or property manager is an additional insured only: <br />1.With respect to Claims arising out of the Named lnsured's occupancy of, or failure to maintain, the Designated <br />Premises; <br />2.For Occurrences at, on, or upon that portion of the Designated Premises that is occupied by the Named Insured <br />and take place during the term of the Named lnsured's lease or occupancy of such Designated Premises; and <br />3.Only when liability for a Claim for which coverage is sought is alleged or determined to be the result of the sole <br />negligence of the Named Insured. <br />However: <br />a.The insurance provided to the additional insured only applies to the extent permitted by law; and <br />b.Will not be broader than that which the Named Insured is required by the lease, or written contract or agreement, <br />to provide for such additional insured. <br />c.Applies solely to Claims first made against the Insured during the Policy Period or Extended Reporting Period, if <br />purchased, if the Named lnsured's coverage is claims-made. <br />This insurance does not apply to any Occurrence which takes place after the Named Insured ceases to be a tenant in <br />the Designated Premises. <br />When coverage does not apply for the Named Insured, no coverage or defense will apply for the additional insured. <br />B.With respect to the insurance afforded to these additional insureds, the following is added to Section Limits Of <br />Liability: <br />MESM 1029 03 21 Page 1 of 2 <br />Docusign Envelope ID: 6D2C2FDD-8D62-85B1-8311-1EA42E180A17
