First Amendment Page 1
FIRST AMENDMENT TO AGREEMENT

This First Amendment to Agreement ("First Amendment") is entered into this 15th day of
June 2026 by the Livermore Pleasanton Fire Department ("City") and The Permanente
Medical Group ("Consultant”), a California Corporation.

Whereas, on July 31st, 2024, LPFD and Consultant entered into Agreement for
oversight of the Livermore-Pleasanton Fire Department controlled substance program,
including the ordering, storing, and dispensing of narcotics identified as Schedule II and
Ill controlled substances by the Controlled Substances Act, and associated clinical
training and support ("Agreement"); and

Whereas, the parties desire to extend the term, replace Scope with Exhibit A-2,
and increase the not-to-exceed amount. The total not-to-exceed amount is increased by
$115,200 to a new, not-to-exceed amount of $190,000.

Now, therefore, in exchange for valuable consideration, the receipt of which is hereby
acknowledged, the parties agree as follows:

1. Except as provided in the body of this Agreement, the hourly rates,
task amounts or travels expenses are intended to be Consultant’s only
compensation for the Services and is inclusive of all costs of labor, licensing,
permitting, overhead and administrative costs, and any-and-all other costs,
expenses, and charges incurred by Consultant, its agents, and employees to
provide the Services

2. Section 3 of the Agreement, “TERM” is amended to read as follows:
“TERM”. This contract shall commence on the date written above and shall
expire on June 30, 2029.

3.
