Should the parties agree to the establishment of such e Trust, the City's contributions on behalf of the employees' will not exceed the amount contributed by the City toward the employee's currant medical, dental and life insurance coverage at the time the 'trust was established. 15.4 Genital Effective May 1, 1990, the City shall contribute an anount not to exceed forty eight dollars (40_00) per month per employee to provide dental care benefits under the City's present Ameritas Life Insurance Plan for the individual employes and eligible dependents. • Effective ii y 1, 1991 said contribution shall increase to an amount not to ekceed fifty two dollars ($52.00), per month per employee. For the purpose of providing of id dental care coverage for eligible regular part-time employees including job share employees who average between twenty (20) and twenty -nine (29) hours per weak, the City Mall contribute an amount not to exceed fifty percent (501) of the city's monthly contribution established for full time employees as set forth above. For regular part-time employees including job share employees who qe between thirty (30) and thirty -nine (39) hours per week, the City contribute an amount not to exceed y -five percent (751) of the City'• monthly contribution established for full-time employees as at forth above. 15.5 ialfi Insurance The City shall contribute an amount necessary to provide 520,000 life and 120,000 accidental death and dismemberment insurance coverage for the individual full -time employee. No later than July 1, 1990 eaid coverage shall increase to $25,000 life and 525,000 accidental death and dismemberment. Effective March 1, Me, employees may purchase, at the employees' own expense and within the limits set by the insurance carrier, additional amounts of life insurance under the existing policy. - 27 -
