Upon qualifying for and receiving Parts A and B of Medicare, <br /> the city shall not be obligated to contribute a monthly amount <br /> in excess of the monthly premium for the Kaiser S -1 supplement <br /> to Medicare Health Plan for employee and one dependent. In <br /> the case where the eligible employee is deceased, the amount <br /> the city is obligated to pay shall be reduced by 1/2 (one- <br /> half). If the spouse remarries, the obligation by the city <br /> shall terminate. <br /> 5.3 Dental Insu ance <br /> Management and confidential employees and their eligible <br /> dependents shall be offered enrollment in the present Ameritis <br /> Bankers Life Nebraska dental plan to include a maximum <br /> lifetime orthodontia benefit of $1,500. <br /> 5.4 Life Insurance. <br /> For the purpose of providing life and accidental death and <br /> dismemberment insurance to management and confidential <br /> employees, the city shall provide coverage equal to twice the <br /> employee's annual salary to a maximum coverage of one hundred <br /> thousand dollars ($100,000). The total monthly premium for <br /> said coverage shall be paid by the city. <br /> 5.5 Lona -Term Disabilitv Insurance. <br /> The city shall provide management and confidential employees <br /> long term disability insurance coverage at no cost to the <br /> employee. The maximum monthly disability insurance benefit <br /> shall be $3,000.00 with a 60 day waiting period. <br /> - 7 - <br />
