DATE: Page 11 of 12 Pages <br /> <br /> <br /> <br />4894-5387-2224 v2 47 <br />o Medical Review Officer report that Covered Employee has verified <br />adulterated or substituted test result; <br />o Admission by Covered Employee that he or she adulterated or <br />substituted their specimen; <br />o Observed possession or wearing of a prosthetic or other device that <br />could be used to interfere with the collection process at time of <br />collection; or <br />o Refusal to wash hands in advance of providing a specimen, after <br />being directed to do so. <br /> Where there is a reasonable suspicion that the Covered Employee is presently <br />under the influence of alcohol or drugs, LPFD shall allow the Covered Employee <br />time to arrange to be safely transported home. The Covered Employee shall not <br />be permitted to transport themself.
