PROPOSAL For Polygraph Examinations submitted to the Pleasanton Police Department Mark Graham / Graham Polygraph 9040 Brentwood Boulevard Suite C Brentwood CA 94513 925.705.1254 SCOPE of WORK I will Administer Law Enforcement Pre-Employment Polygraph Examinations according to industry standards. Each examination will comply with the standards set forth by the American Polygraph Association (APA), American Association of Police Polygraphists (AAPP), and the Association for Testing and Materials (ASTM). I will carry Commercial and Professional Liability Insurance. I will provide modern, state-of-the-art polygraph equipment. I use the Lafayette 4000 computerized polygraph instrument. I will provide all materials required for each examination including a digitally retained copy of each exam. Exams will be conducted in my business office, located in Brentwood. This office is modern, has plenty of parking and easy highway access. All polygraph examinations will be digitally recorded (video). The digital recordings and reports will be retained for a period of no less than three years. A written report will be forwarded to the client within two business days of the examination. A verbal summary of the report of the examination results will be provided within 24 hours, at the client’s request. All polygraph examinations will be held in the strictest confidence. All polygraph reports/results will be considered the property of the City of Pleasanton Police Department. I agree to provide expert testimony, for a fee, at the request of the Pleasanton Police Department. When necessary, I am willing to adjust my business hours to accommodate academy students, lateral applicants and candidates from outside of the Bay Area.
